Health & Prescription Drug Insurance Employees are eligible the first of the month following 30 days of employment.Physicians Plus HMO and POS plan options are available.Divine Savior Healthcare heavily subsidizes the cost of individual and family coverage.Health plan includes coverage for prescription drugs.Prescriptions may be filled at DSH Pharmacy at a discount.
Dental Insurance Employees are eligible the first of the month following 30 days of employment.Coverage for preventive, routine and major dental procedures and dependent orthodontia.Divine Savior Healthcare heavily subsidizes the cost of individual and family coverage.
Vision Insurance Employees are eligible the first of the month following 30 days of employment.Options of Materials Only or Materials and Vision Exam coverage available.
Life Insurance Employees are eligible the first of the month following 30 days of employment.Group life, accidental death and dismemberment insurance provided by Divine Savior Healthcare. This benefit is 100 percent of your salary rounded to the next $1,000.Supplemental and Dependent Life Insurance available at group rates for employee, spouse and children.
Income Protection Employees are eligible the first of the month following 30 days of employment.Voluntary plan paid by payroll deduction.
Flexible Benefit Plan - Dependent Care and Unreimbursed Medical Expense Account  Employees are eligible the first of the month following 30 days of employment. Enrollment is conducted on an annual basis.Allows employee payments for health and dental coverage to be deducted on a pre-tax basis in accordance with IRS regulations.Allows employees to deduct expenses for child care services and unreimbursed medical, dental, and vision costs on a pre-tax basis in accordance with IRS regulations.
401(k) Retirement Plan Employees are eligible at age 18. Enrollment is conducted on January 1, April 1, July 1 and October 1.Employees may shelter up to the maximum amount allowed by law pretax for retirement. After one year of service and 1000 hours worked, Divine Savior Healthcare will contribute an amount equal to 2% of your salary to your plan. Additionally, the company will match $.50 for each $1.00 you contribute up to 6%of your covered compensation.
Paid Time Off Includes time off generally associated with vacation, holiday and personal day programs.Begins accumulating upon employment; cannot be taken until after six months of continuous employment.Employees earn PTO hours based on regular hours worked. Full-time employees who work 80 hours per pay period earn a total of 168 hours (or 21, 8-hour days) of PTO in their first year.Employees are eligible to earn additional time after their 5th and 15th years.
Disability Bank Begins accumulating upon employment; cannot be taken until after six months of continuous employment.Time off with full-pay beginning on third consecutive day of employee's illness or on first day of hospitalization or outpatient surgery. Maximum of 72 hours earned per year.

Tuition Reimbursement Employees are eligible after six months of employment.Divine Savior Healthcare reimburses employees working 20 or more hours per week for 75% of the cost of tuition for degree programs in healthcare careers. Maximum reimbursement is $3,500 per school year. Books and other fees are covered.Reimbursement is only given if the employee completes the course with a grade of “C” or better.
Scholarship/Loan Employees are eligible upon application.Renewable scholarship/loan up to $1,000 per academic year for five years. Must be a full-time student. Two new awards are given each year.
Financial Services Employees are eligible upon employment.Payroll checks are directly deposited into any checking and/or savings account(s) at any financial institution.
Health Services Pre-employment physical, lab work and TB test.Annual TB screening, flu shot and Hepatitis B vaccinations are also available.Employee Fitness Gym—Employees are eligible upon employment. An annual $60 fee gives employee access to rehab equipment and locker rooms before and after work hours.Employee Assistance Program—Employees are eligible upon employment. Employee Assistance Program offers free counseling and seminars on personal issues.
Service Awards Eligible upon completion of 5, 10, 15, 20, 25 and beyond years (in five-year increments) of service.A gift of recognition for years of service is given.
